Sentara Norfolk General Hospital is seeking a Community Health Worker (CHW) who is committed to creating meaningful impact in the lives of individuals and families across Hampton Roads.
About the Role
As a Community Health Worker, you will serve as a vital bridge between the hospital and the community. You will connect individuals to resources, advocate for underserved populations, and help reduce barriers to care—empowering people to take control of their health and well-being.
You’ll work closely with clinical teams, social workers, and community partners to support individuals navigating complex social and medical needs. This is an opportunity to work on the front lines of health equity, driving change from within one of Virginia’s most respected healthcare institutions.
Key Responsibilities

Establish trust with patients and community members through outreach, education, and follow-up.

Connect individuals with services including primary care, transportation, housing, food access, and mental health support.

Collaborate with healthcare providers to develop and implement care plans tailored to patients’ needs.

Conduct home visits and attend community events to provide support and information.

Advocate for patients and identify systemic barriers impacting health outcomes.

Track and document outcomes to demonstrate impact and ensure continuity of care.
What We’re Looking For

Experience working in community health, social services, outreach, or related settings.

Knowledge of Hampton Roads/ Tidewater Region and surrounding communities is a plus.

Strong communication, cultural competency, and problem-solving skills.

Ability to work independently and collaboratively within a hospital setting.

Passion for social impact, health equity, and serving vulnerable populations.
Preferred Qualifications:

Associate’s or Bachelor’s degree in Public Health, Health Sciences, Social Work, or related field.

CHW certification or willingness to complete certification (training support available).
