Responsibilities
Sentara Medical Group is looking for an innovative, compassionate APP to join our growing palliative care program. This APP role will develop and support the building of a new home-based palliative care program. This individual will be part of an interdisciplinary team consisting of RNs and an MSW who will collaboratively provide palliative care services. This role will include both home and telehealth based palliative care visits. Position Highlights: ●Locations: Virginia Beach, Norfolk, Chesapeake ●Monday- Friday; 1-2 days a week completing home visits with remaining days telehealth ●Home visit are 1 hour long, 3-4 patients a day ●Performing palliative care consultations and follow up visits for patients with acute palliative care needs including symptom management and complex medical decision making ●Facilitating family meetings related to current medical decision making and advance care planning ●Actively participating as a member of an interdisciplinary team to ensure that palliative care is provided in a manner which addresses pain and symptom management; psychosocial/spiritual support; and caregiver support.
