Overview
Sentara Home Care is currently hiring a full time Care Transition Coordinator for the Greater Peninsula.
The Care Transition Coordinator is responsible for the identification and assessment of home care needs of patient population and coordination of these services to ensure a smooth transition to the next site of care.
The Care Transition Coordinator will be based out of the Greater Peninsula Hospitals.
Shift: Monday – Friday (8am-5pm)
Requirements:
1 year of Home Care/Hospice, Discharge Planning OR Case Management experience required.
Licensed RN, LPN, RT or BSW required.
