Sentara Home Care Services Chesapeake is hiring a full time Home Health Aide!
Schedule: 8am-5pm Monday- Friday
Overview
Utilizes safety precautions when caring for patients with specialized needs (oxygen, IV, tubes, drains, wounds and dressings). Monitors and records elimination (monitors urine and bowel elimination) patterns and reports any concerns to the appropriate clinician. Provides care to prevent pressure ulcers (minimize pressure to body parts by turning, positioning and skin care). Completes all departmental documentation, (i.e. logs, flow sheets, and patient medical records. Recognizes deviations and changes in vital signs and reports to the appropriate clinician.
Provides specific care that supports psychosocial functioning. Demonstrates active listening skills and pays close attention to patient's verbal/nonverbal messages. Provides emotional support to include reassurance, acceptance and encouragement during times of stress.
Provides specific care to support the physical functioning of the patient. Assists patients in feeding. Assists in the delivery of activities of daily living (ADLs). Utilizes infection control precautions when providing care. Assists patient with transfer as required using assistive devices as needed and assists with ambulatory activities. Facilitates safe transfer and mobility of patients and follows high risk precautions for identified patients. Demonstrates proper body mechanics for transferring patients. Nutritional support to include meal preparation taking into consideration special diets and/or food allergies. Assists in the collection of information about the patient and the environment for use in promoting and maintaining patient safety. Collects and launders soiled linens as needed or requested and stores clean linens appropriately. Ensure that the work area is neat and tidy and disposes of waste materials in an appropriate manner.
