Job Description
The Wound Ostomy Continence Nurse role involves comprehensive patient assessment, developing individualized care plans, and utilizing advanced nursing techniques to promote healing, prevent complications, and enhance the patient's quality of life. The WOCN collaborates with multidisciplinary teams, including physicians, physical therapists, and dietitians, to deliver holistic care tailored to the unique needs of each patient. Key responsibilities include: Wound Care: Assess and manage acute and chronic wounds, including pressure ulcers, surgical wounds, diabetic foot ulcers, and other complex skin conditions. Ostomy Care: Provide pre- and post-operative care to patients with ostomies including teaching patients about stoma care, appliance selection, and skin protection. Continence Management: Assist patients with urinary and fecal incontinence, providing strategies to manage these conditions through both non-invasive and invasive treatments. Educate patients and caregivers on proper use of continence products and techniques. Patient Education and Counseling: Offer individualized education to patients and families on self-care techniques, product use, and lifestyle modifications. Address emotional and psychological concerns related to wound, ostomy, and continence care. Collaboration and Advocacy: Work closely with physicians, nursing staff, and other healthcare providers to coordinate care and advocate for the patient’s needs. Documentation and Compliance: Maintain accurate patient records, ensuring proper documentation of care plans, interventions, and progress. Basic Life Support (BLS) required within 90 days of hire.
EDUCATION: ADN, BSN OR MSN
LICENSURE / CERTIFICATIONS:
REQUIRED: Valid RN License to practice in Virginia,
Certified Wound Ostomy Nurse (CWON) certification.
EXPERIENCE: At least 1 yr of CWON experience preferred
