Sentara RMH Medical Center is now hiring a Case Manager (MSW Social Worker). This is a position that will be responsible for the provision of medical social work services to patients receiving care in the Hahn Cancer Center.
Education
Master’s Degree – Social Work
Certification/Licensure
- Basic Life Support (BLS) – Certification – American Heart Association (AHA) RQI within 90 days of hire
- Certified Case Manager (CCM) – Certification – Commission For Case Manager Certification (CCMC) within 1 year of hire
Experience
- 1 year of Case Management or other related experience
Duties/Responsibilities
- Responsible for the provision of medical social work services to patients receiving care in the hospital setting.
- Assesses the social, psychological, cultural, environmental, and financial situations, as well as disposition needs for each referred patient.
- Collaborates with patients, families, healthcare team members, and community organizations, and using strong advocacy skills identifies needs and resources in the hospital and community.
- Develop and evaluates effective transition plans taking into consideration the client diagnosis, prognosis course of treatment, past and present services, short-term and long-term goals, provider options, and/or available health care benefits as appropriate to the continuum of care.
