Sentara Health is seeking to hire a qualified individual to join our team as a Care Coordinator for the Roanoke region!
Standard Working Hours: Monday – Friday, 8am – 5pm.
This is a Full-Time position with day shift hours and great benefits!
The position requires both in-person face-to-face assessments and remote telephonic assessments in and around Roanoke, Martinsville, and surrounding communities.
*Applicants must reside within driving distance.
Responsibilities :
Performs a variety of casework duties and provides case management services to patients, families, and designated caregivers. Must develop, participate and monitor multidisciplinary collaboration of services to patients where appropriate. Assist adult patients and their families with personal and environmental difficulties associated with medical condition up to and including at time of terminal illnesses.
Education
- Must possess a Bachelor's degree in Health & Human Services or one of the following related Fields: Art Therapy
