Sentara Martha Jefferson Hospital is hiring a Registered Nurse Inpatient Case Manager for a Full Time, Day shift position in Charlottesville, VA!

The Integrated Case Management team at Sentara Martha Jefferson has 16 members including MSW and RN Case management staff. This is a collaborative environment with a patient- first mind set. The average cases per day are 15, and a strong sense of urgency is preferrable.

The hours are primarily between 8am and 430pm Monday to Friday with a rotating weekend schedule every 8 weeks.

The Registered Nurse- Inpatient Case Manager Responsible and accountable for the provision and facilitation of comprehensive care management, services, and quality outcomes for patients across the healthcare continuum, primary focus on the patient transition to the appropriate level of care while facilitating clinical appropriateness and LOS. Directly support those in-hospital patients identified in need of comprehensive case management. Promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions. Develops and/or implements a comprehensive care plan based on assessment and evaluation to include the physical, mental, and psychosocial needs of the inpatient/caregiver. Functions in one of the following practice settings: Hospitals Only.

Education

• Degree or Diploma in Registered Nursing (required)

• Bachelor of Science Nursing- BSN or MSN (Preferred)

• Will be required to complete the BSN within 5 years of hire if not obtained prior.

Certification/Licensure

• Registered nursing License (Required)

• BLS required within 90 days of hire

• Case Management (ACM, CCM, CCCTM, CMAC, or CMGT-BC) certification is required within one year of eligibility.

Experience

• 1 year of Acute care nursing experience required.

• 5 years of adult acute (hospital based) nursing experience preferred.

• Experience with community resources is preferred.

• Previous experience in Case Management preferred.
