Sentara Health Plans is hiring a Temporary Care Coordinator for the Northern/Central Virginia area!
Position Status: This temporary position is set to last a minimum of 3 months but has the possibility of extending.
Position Location: Hybrid in Northern/Central area. Must be able to travel 3-4 days a week for in-person assessments.
Area includes: Culpeper, Spotsylvania, Louisa, Rapphannock, Fredericksburg, Woodbridge, Prince Edward, Farmville, Manassas
This position requires both in person face-to-face assessments and remote telephonic assessments! Applicants must be located in Western/Roanoke area.
Standard Working Hours: 8:00AM to 5:00PM
Overview
Performs a variety of casework duties and provides case management services to patients, families, and designated caregivers. Must develop, participate and monitor multidisciplinary collaboration of services to patients where appropriate. Assist adult patients and their families with personal and environmental difficulties associated with medical condition up to and including at time of terminal illnesses
