Sentara Health is hiring for a Community Health Worker in Hampton Roads (Norfolk, Virginia Beach, Newport News)!
Status: Full-time position (40 hours) with a contract for 2 years.
Standard working hours: Dayshift, 8am to 5pm EST; With weekend or evening hours, as needed
Location: With a remote work opportunity. Will travel to local churches in Hampton Roads (Norfolk, Virginia Beach, Newport News) for clinical research/studies (Blood Pressure Check) as needed. Reports to Sentara Health Research Center at Centre Pointe in Virginia Beach, VA, as needed.
Job responsibilities:

Works in collaboration with the research Manager and other research team members to provide comprehensive and coordinated care.

Engages in interventions to improve self-management skills and improve quality of life for patients needing workup for high blood pressure diagnosis.

Provides education and behavior modification strategies and provides social support to patients and families.
-
Acts as a patient advocate and healthcare navigator in assisting with resource allocation and management.
Required qualifications:

Driver's License – Certification – Other/National REQUIRED

HS – High School Grad or Equivalent REQUIRED
Preferred qualifications:

Experience working in a community-based setting preferred.

Knowledge of medical terminology preferred
